  • Een droge huid is een huid die niet genoeg water kan vasthouden. Hier zijn twee dingen bij betrokken: het water dat in de buitenste laag wordt vastgehouden, en de vetten die verhinderen dat het ontsnapt.
  • De buitenste huidlaag houdt water vast met een lipide “mortel” van ceramiden, cholesterol en vetzuren, en houdt dit vast binnen de cellen met natuurlijke vochtinbrengende factor.
  • Meer van dezelfde vochtinbrengende crème werkt vaak niet meer doordat de oorzaak aanhoudt: heet water, agressieve reiniging en droge, verwarmde binnenlucht.
  • Bevchtigers trekken vocht aan, emolliëntia verzachten, occlusiva verhinderen dat het ontsnapt. Bevochtigers op zich kunnen het waterverlies via de huid verhogen, daarom werken de drie het beste samen.
  • In een Nederlandse studie onder 5.547 personen van middelbare en oudere leeftijd, 60% had een droge huid, en het dagelijks gebruik van crème werd in verband gebracht met minder een droge huid in gelokaliseerde plekjes.
  • Raadpleeg een huisarts als de huid barst of bloedt, er geïnfecteerd uitziet, pijnlijk is of zo erg jeukt dat het uw slaap verstoort.

Elk jaar, naarmate de avonden korter worden en de verwarming aangaat, gebeurt hetzelfde. De huid die de hele zomer in orde was, begint na het douche strak aan te voelen. Handen worden ruw. Scheenbenen schilferen. Je grijpt naar meer vochtinbrengende crème, en dat helpt een uur, en daarna niet meer.

Dat laatste deel is waar dit stuk over gaat. Als je meer hydrateert en je huid er niet beter op wordt, is de vochtinbrengende crème niet het hele probleem. Dit is wat er onderhuids aan de hand is en wat werkelijk helpt.

Droog of gedehydrateerd?

Je leest vaak dat een “droge” huid tekort aan talg heeft en een “gedehydrateerde” huid tekort aan water. Het is een mooi onderscheid, maar dermatologen trekken die grens niet zo scherp.

De Amerikaanse Academie voor Dermatologie beschrijft een droge huid als wat er gebeurt wanneer de huid te snel vocht verliest, en vermeldt dat deodorantzeep en agressieve reinigingsmiddelen oliën en vetten van de huid kunnen strippen tijdens dit proces. Er staat alsook in dat een uiterst droge huid zoveel vocht heeft verloren dat deze merkbaar is uitgedroogd. Cleveland Clinic omschrijft een droge huid simpelweg als een huid die niet genoeg vocht bevat om zacht aan te blijven voelen.

De nuttigste manier om erover na te denken is dus als twee taken. De ene is het vasthouden van water in de buitenste huidlaag. De andere is voorkomen dat dat water ontsnapt. De vetten in je huid doen de tweede taak, en wanneer deze op zijn, mislukt de eerste taak ook. Dat is waarom “water toevoegen” en “olie toevoegen” alle beide een half antwoord zijn.

Hoe je huid water vasthoudt

De buitenste laag van je huid, de hoornlaag, wordt vaak omschreven als bakstenen en cement. De bakstenen zijn afgeplatte huidcellen. Het cement is een mix van lipiden.

De mortier. Een recensie in de Journal of Lipid Research beschrijft de lipidenmatrix tussen de cellen als voornamelijk bestaande uit ceramiden, cholesterol en vrije vetzuren, en zegt dat het deze matrix is die de barrière vormt tegen water. Een overzicht van vochtinbrengende middelen in Klinische Geneeskunde & Onderzoek stelt de conclusie duidelijk: het verlies van deze intercellulaire lipiden beschadigt de waterbarrière en leidt tot een droge huid.

Het water in de stenen. In de huidcellen bevindt zich natuurlijke vochtinbrengende factor, of NMF, een mengsel van kleine, wateraantrekkende moleculen. Een overzichtsartikel in Dermatologische Therapie beschrijft het als gevormd in de cellen uit de afbraak van een eiwit dat filaggrine wordt genoemd. De Klinische Geneeskunde & Onderzoek recensie voegt eraan toe dat ongeveer de helft van NMF bestaat uit aminozuren, en de rest uit zouten zoals lactaat, ureum en elektrolyten, en dat de productie van NMF direct gerelateerd is aan de luchtvochtigheid om je heen.

Het lek. Water verdampt altijd door de huid. Dermatologen noemen dit transepidermaal waterverlies, of TEWL. Hetzelfde Dermatologische Therapie Uit de recensie blijkt dat de processen die een goede vochtbarrière opbouwen zelf worden beïnvloed door hoe gehydrateerd de huid is, en dat deze processen vaak worden verstoord door omgevingsfactoren, wat resulteert in een droge, schilferige huid. Met andere woorden, een droge huid maakt het voor de huid moeilijker om precies die barrière te herstellen die zou moeten voorkomen dat de huid droog is.

Waarom meer moisturizer niet meer werkt

Als je steeds meer crème blijft opsmerken en je huid blijft strak en ruw aanvoelen, zit hier meestal een of meer van de volgende oorzaken achter.

Je gezichtscrème doet slechts één van de drie taken

Bevochtigers, zoals glycerine en hyaluronzuur, trekken water aan naar de buitenste laag van de huid. Maar de Klinische Geneeskunde & Onderzoek Uit recensies blijkt dat humectanten ook water uit de diepere huid naar het oppervlak kunnen trekken, waar het gemakkelijk verdampt, wat de reden is dat ze zo vaak worden gecombineerd met occlusieven. Een lichte, humectantrijke lotion kan heerlijk aanvoelen bij het opbrengen en je huid een uur later alsnog in dezelfde staat achterlaten.

Het is te dun voor hoe droog je huid is

De American Academy of Dermatology zegt dat zalven en crèmes meer vocht toevoegen aan de huid en effectiever zijn dan lotions. Huisarts, de patiëntensite van de Nederlandse huisartsen, zegt dat een dunne bodylotion of bodymilk vaak niet vet genoeg is voor een droge huid. Het eerste advies is een eenvoudige basiszalf, zoals cetomacrogolcrème, tweemaal daags of vaakker over het hele lichaam. U kunt dit zonder recept kopen bij een drogist of apotheek. Een Duits standpunt Bij een droge huid geldt als vuistregel: hoe droger de huid eruitziet, hoe hoger het vetgehalte van het product moet zijn.

De schade treedt nog steeds op

Je kunt niet hydrateren tegen iets in dat je huid elke dag stript. Een recensie in Dermatologische Therapie beschrijft hoe agressieve oppervlakteactieve stoffen in reinigingsmiddelen huideiwitten en -lipiden kunnen beschadigen, wat leidt tot een strak gevoel na het wassen, droogheid, beschadiging van de huidbarrière, irritatie en zelfs jeuk. Uit diezelfde beoordeling bleek dat oplossingen met een hoge pH, zelfs zonder oppervlakteactieve stoffen, de buitenste huidlaag kunnen laten opzwellen en de lipiden ervan kunnen veranderen. Heet water draagt hier nog extra aan bij, zoals blijkt uit het onderstaande gedeelte over het wassen.

De lucht om je heen is droog

Binnenlucht is droger zodra de verwarming aan staat. Meer daarover hieronder.

Het is niet alleen een droge huid

Soms is hardnekkige droogheid een symptoom van iets anders. De American Academy of Dermatology noemt atopische dermatitis, psoriasis en nierziekte als enkele van de aandoeningen die een te droge huid kunnen veroorzaken. Dat is een taak voor een huisarts, niet voor een nieuwe crème. Raadpleeg wanneer naar de huisarts.

Wat de herfst met je huid doet

Een recensie in de Tijdschrift van de Europese Academie voor Dermatologie en Venereologie keek naar hoe luchtvochtigheid en temperatuur de huid beïnvloeden. Er werd geconcludeerd dat een lage luchtvochtigheid en lage temperaturen leiden tot een algemene afname van de huidbarrièrefunctie, en de huid gevoeliger maken voor irriterende stoffen en allergenen. Er wordt op gewezen dat mensen in Noord-Europa, die worden blootgesteld aan strenge winters, een droge en jeukende huid kunnen ervaren.

Binnen is een deel van het probleem. Thuisarts legt uit dat de lucht binnen in de winter droger is omdat het binnen zoveel warmer is dan buiten. Als je dat combineert met het feit dat de NMF-productie afhangt van de luchtvochtigheid, is de verwarming die aan staat een betrouwbare trigger.

Voor lezers in Nederland is er ook lokale data. De Rotterdam Study, gepubliceerd in de Journal of the American Academy of Dermatology, lieten artsen de huid beoordelen van 5,547 middelbare en oudere volwassenen. 60% had een droge huid. Buitentemperaturen waren een van de factoren die hier significant mee samenhangen, samen met leeftijd, vrouw zijn en eczeem. En dagelijks gebruik van crème was geassocieerd met minder gelokaliseerde droge huid. Dat is een associatie van een enkel moment in tijd, geen bewijs dat de crème dat heeft veroorzaakt, maar het wijst in dezelfde richting als al het andere hier.

Leeftijd speelt een rol. De American Academy of Dermatology merkt op dat de huid vanaf de middelbare leeftijd minder talg aanmaakt, haar natuurlijke olie, en dat tegen de leeftijd van 60 jaar bijna iedereen een droge huid heeft.

Bouw voordat je oplicht: gelaagdheid

Dit is de kern van de Junita-aanpak. Voordat je denkt aan verhelderen, gladmaken of iets anders, moet je ervoor zorgen dat de barrière weer water vasthoudt. Voor een demente huid betekent dat alle drie de taken vervullen.

A few things from the research are worth knowing:

  • Glycerine is one of the best-studied humectants. A review in the British Journal of Dermatology describes it improving the hydration and barrier function of the outer skin layer, and says its hydrating effect is critical in skin made worse by dry, cold conditions, such as winter dryness.
  • Petrolatum is the classic occlusive. The Klinische Geneeskunde & Onderzoek review reports that at a minimum of 5% it can reduce water loss from the skin by more than 98%.
  • Ureum, according to the Duits standpunt, has by far the best evidence for treating dry skin, and works better combined with other natural moisturising components and ceramides.
  • The skin’s own lipids. De Klinische Geneeskunde & Onderzoek review cites research showing that mixtures of ceramides, cholesterol and free fatty acids in a 3:1:1 ratio can speed up barrier repair.

You do not need three separate products. Many creams combine two or three of these jobs in one jar, and reading the ingredient list tells you which. The lines are not hard ones, either: fatty alcohols such as cetyl alcohol soften the skin, and a review in Skin Therapy Letter also lists them among the occlusives, so a cream without petrolatum or dimethicone can still do a little of the third job. What matters is that something is holding the water in, not only pulling it up.

If your skin is dry, the American Academy of Dermatology advises you to stop using skin care products that contain alcohol (except hand sanitiser), fragrance or retinoids. Brightening and resurfacing can wait until the barrier is back.

De wasbeurt is net zo belangrijk als de crème

This part is easy to overlook, and it matters as much as what you put on afterwards.

  • Keep it short and not too hot. The American Academy of Dermatology recommends limiting baths and showers to five to ten minutes in warm water. Thuisarts says the same five to ten minutes, and no warmer than 37 or 38 degrees, and adds that long, hot showers rinse away the skin’s own fatty layer.
  • Heat makes cleansers harsher. In a study in Contact Dermatitis, detergent solutions caused more skin irritation, measured by water loss, redness and dryness, at 40°C than at 37°C.
  • Use less cleanser, and only where you need it. The AAD advises using a gentle cleanser, not enough to make a thick lather, and only where you need it, such as the armpits and groin. Thuisarts says to use as little shower gel, soap and shampoo as you can.
  • Shower less often. Thuisarts suggests no more than once a day, and says a shower every day is not necessary.
  • Pat, do not rub, and moisturise straight away while your skin is still damp. The AAD recommends applying moisturiser when skin is still damp after a shower or bath, and after washing your hands.

Who this approach is for

It is not for skin that is cracked and bleeding, weeping, infected or painful. That needs a GP, not a change of routine.

Hoe te beginnen, zachtjes

  1. Turn the shower down and shorten it. Warm, not hot, five to ten minutes.
  2. Use less cleanser, and only where you need it.
  3. Pat dry and moisturise within a few minutes, while your skin is still damp.
  4. Move from lotion to cream, or from cream to a richer cream or ointment, if your skin is very dry.
  5. Check your moisturiser covers more than one job: a humectant near the top and an oil, butter or occlusive alongside it.
  6. Moisturise your hands after every wash, and wear gloves for washing up and cleaning.
  7. Add moisture to the air. The AAD suggests running a humidifier in the bedroom while you sleep, and cleaning it regularly.
  8. Put aside products that contain retinoids, fragrance or alcohol until your skin is comfortable again.
  9. Make it daily. Every day, not only on the days your skin feels bad.

Wanneer naar de huisarts

Most dry skin can be managed at home. But see your GP (in the Netherlands, your huisarts) if:

  • You have followed the advice and your skin is not getting better, or it is getting worse. Thuisarts says your GP can check whether it is eczema.
  • Your skin itches constantly and it interferes with your sleep or daily life.
  • It looks infected (red, warm or swollen), is painful to touch, or you develop a rash.
  • It has cracked open or is bleeding. Cleveland Clinic notes that severely dry skin can crack and bleed, and that open cracks let in germs that can cause infection, so do not wait for it to look infected.

The American Academy of Dermatology also recommends seeing a dermatologist if your skin stays dry after trying these steps, because overly dry skin can be a sign of an underlying condition such as atopic dermatitis or psoriasis.

Glycerin, hyaluronic acid and shea butter are all on the American Academy of Dermatology’s list of what to look for in a cream for dry skin. For the third job, holding water in, every one of them contains cetyl alcohol, one of the fatty alcohols the Skin Therapy Letter review above lists among the occlusives. Despite the name, it is a white solid, not the alcohol (ethanol) that labels list simply as “alcohol”. Its seal is much lighter than petrolatum’s, and none of our creams contains petrolatum, dimethicone or another classic occlusive, so on very dry patches a plain base cream or ointment on top is still the right call. Most of them also list alcohol, and most list at least one of the fragrance allergens the EU requires to be named on a label. So if your skin is dry and reactive, the advice above applies to ours as much as to anyone’s: put them aside until your skin has settled. We would rather say that plainly than pretend otherwise.

What is the difference between dry and dehydrated skin?

Beauty writing often says dry skin lacks oil and dehydrated skin lacks water. Dermatologists do not separate them that neatly. The American Academy of Dermatology describes dry skin as skin that loses water too quickly, and very dry skin as noticeably dehydrated. In practice both water and the skin’s fats are involved, because the fats are what hold the water in.

Why does my skin still feel dry even though I moisturise?

Usually because something is still drying it out, such as hot showers, harsh cleansers or dry indoor air, or because the moisturiser only attracts water without holding it in. Humectants on their own can draw water to the surface where it evaporates, so they work best combined with oils, butters or occlusives.

Is hot water bad for dry skin?

Long, hot showers make dry skin worse. The American Academy of Dermatology recommends five to ten minutes in warm water, and Thuisarts advises no warmer than 37 or 38 degrees. Research has also found that cleansers irritate skin more at higher water temperatures.

Does central heating cause dry skin?

It contributes. Heated indoor air in winter is drier, and a review of the evidence found that low humidity and low temperatures reduce skin barrier function. The skin’s own natural moisturising factor also depends on the humidity of the air. A humidifier in the bedroom can help.

Is a cream or a lotion better for dry skin?

A cream or ointment. The American Academy of Dermatology says they add more moisture and are more effective than lotions, and Thuisarts says a thin body lotion is often not greasy enough. The drier your skin, the richer the product should be.

What ingredients should I look for?

A humectant such as glycerin, hyaluronic acid or urea, alongside emollients such as plant oils and butters, and ideally an occlusive such as petrolatum or dimethicone. Urea has particularly strong evidence for dry skin, and glycerin is one of the best-studied humectants.

Is dry skin more common as you get older?

Yes. Skin makes less sebum from middle age, and the American Academy of Dermatology says nearly everyone has dry skin by 60. In the Dutch Rotterdam Study, 60% of 5,547 middle-aged and older adults had dry skin.

Could my dry skin be eczema?

It can be. If your skin does not improve with good care, gets worse, cracks, bleeds or itches enough to disturb your sleep, see your GP. They can check whether it is eczema or another condition that needs treatment.
